Some other countries have similar rates of incarceration for political crimes. Violent criminals are pretty rare in practice, or civilization would never work.

Use a big plastic mug (hard to counterfeit and wimpy as a weapon), full of a tasty, painless poison that only works if you drink the full 20 ounces. Even if he splashes the guard with it, the guard won't swallow a lethal dose.

You don't want an inmate to use their hands? Restrain them. You don't want them to hide something? Then don't look away until they're dead.
Aug 8, 2016 at 11:32 comment added Philipp Replace the pill with a dummy made from harmless prison supplies, use sleight of hand to slip it into the guard's pocket and then steal it back from them while being searched, hide it in the ventilation shafts... a creative prisoner will find a way to smuggle that pill out of there.

If the pill can't be accounted for, then nobody leaves the room. Also, it doesn't say you have to leave them alone in the room.
Aug 8, 2016 at 11:24 comment added Philipp And what do they do when they don't find it on the inmate? Maybe the inmate hid it somewhere in the room where the guards don't think of looking. Then there are now two pills in the room and the next inmate can pilfer it. There is just too much that can go wrong in that setup.

Aug 8, 2016 at 11:20 comment added Philipp I see a problem with people going to the room, stealing the pill, saying they changed their mind and use it to poison another inmate. The guards would of course notice that the pill is missing after the visit and not let the inmate leave. The inmate could then convince them that it wasn't there in the first place.
