One of the easier ways to contaminate the water source of your enemy is to throw a corpse in the well. That way you could give them cholera among other diseases. However, if your enemy has running water, like the Romans, your chances of poisoning the water supply is next to zero.

Use your people as hosts for the disease-in-progress, and help the most deadly strains of the disease to spread (by leaving corpses to rot in the streets) while cutting off the strains that are too obvious (by burning alive anyone discovered to have the disease). In a few generations, you'll have a certifiable biohazard that your people are partially resistant to. I'm sure a sufficiently evil villain will have no problem with this scheme...

That is an academic research piece, not a conspiracy theory.

And Brits used it (knowingly, by contaminating blankets with blood of smallpox patients and sending them as presents to local chiefs) to effectively exterminate huge populations of native indians In north America.

A few smallpox infected folks would spread the disease in the entire population. Better yet, plague or black death! Also, use biologically tampered arrows/bullets. Use rabid dog saliva on your arrows/bullets and no psychic healer will ever be able to heal the infected. That's not too much of a tech advanced thing really!

Method 2: landmines, that sounds interesting, but, ah, well, even my Cannon Master doesn't know what that is. So, ah, you know.. Method 1: other evil countries want a piece of me too, so I don't think I should mobilize all my troops and leave my back open for stabbing.
