In a pre-modern society you will necessarily have hundreds of different cultures in the pieces of land separated by natural barriers. Only in a very modern world with cheap long-distance travel and instant long-distance communication can you hope to have only a small number of cultures.

However, if you take the cold war as an example, there could be two dominant world superpowers... but that does not exactly constitute only two cultures!
